These forms can be faxed (512) 251-6970 or emailed (info@doggydayout.com) back to Doggy Day Out. Filling out these forms does not guarantee a reservation. You will receive a confirmation of your reservation from Doggy Day Out.
Your Doggy Daycare Camper Will Need:
- A leather or nylon collar with tags
- Current shot records must include:
Rabies Vaccination
DHLPP
Bordatella (Kennel Cough)-Boostered every 6 months if administered intranasal or yearly if injectable.
Dogs must currently be on prescription heartworm and flea prevention and have a negative fecal and heartworm test.If your vet has a different vaccination protocol, please ask them to include a note stating their protocol with your vaccination paperwork.
Vaccination records must be faxed to us before your dog arrives at (512) 251-6970Note: All dogs over the age of six (6) months must be spayed or neutered, unless they are registered and are currently being campaigned as show or field trial dogs
Your Overnight Boarding Dog Will need:
- A leather or nylon collar with tags
- Food from Home
Because we believe it is better not to change your dog's diet and environment at the same time, we will happily feed your dog's food from home at no extra cost. It is requested that food be brought in a sealed container or in pre-portioned "baggies". Please do not bring a scoop or cup. We provide Blackwood Canine Nutrition dog food upon your request for $3.00 a day.
- Durable toys such as Nylabone, Kong, or Goodie Ship. For safety reasons, Doggy Day Out does not permit rawhide treats of any kind.

Our office is closed to the public for pick-up and drop-off on the following Holidays. Please rest assured that our kennel technicians are on-site taking care of your pets.

- New Year's Day
The day prior to Thanksgiving, Christmas Eve and New Year's Eve we will close at 2:00 pm. The day after the above holidays, we will reopen at 10:00 am, unless it is a Sunday, in which case we will open at 4:30 pm.
Daycare is not offered on the day before or after the above holidays.
Cancellation Policy:
We require 48 hours advance notice for cancellation of non-holiday reservations. Cancellations with less than 48 hours notice will be charged as follows: 1/2 nights reserved or $75 which ever is less. If reservation was for one night then customer must pay for that night. The fee for an early pick up is calculated at 50% of the remaining nights reserved.
Holiday Cancellation Policy:
July 4th, Thanksgiving***, Christmas***, New Years
We require 72 hours advance notice for cancellation of holiday reservations. Cancellations with less than 72 hours notice will be charged as follows: 1/2 nights reserved or $75 which ever is less. If reservation was for one night then customer must pay for that night.
***Reservations for Thanksgiving and Christmas require a $50.00 per dog non-refundable deposit.
